Family Survey
Please participate with this important survey for schools in the Los Angeles Unified School District .
This information is important to St. Dominic School so that we can receive E-rate funding. E-rate funding will allow our school to receive a discount on local phone service, long distance service, and internet access.
Our discount depends on the number of families that respond to this survey. Your family name is not needed on the survey but please put your complete address and phone number.
We need to submit the information to the LAUSD. Please return the survey by this Friday, Sept. 23.
